Responsibilities
- Provide direct, evidence-based physical therapy services to home health patients, including evaluation, treatment planning, and progress documentation.
- Collaborate with an interprofessional team to develop and implement the patient’s comprehensive plan of care.
- Deliver therapeutic interventions and modalities; manage rehab equipment (splints, orthoses, prostheses, wheelchairs) and ADL equipment as needed.
- Assess home environments for safety, recommend adaptations, and educate patients and caregivers on safe and effective home care.
- Complete OASIS documentation and other required records; ensure compliance with regulatory standards and agency policies.
- Work independently with minimal supervision, traveling to patient homes within the assignment area and coordinating with team members to optimize outcomes.

Requirements
- Massachusetts Physical Therapist license; active and current (within the past year acceptable per policy)
- Home Health Certification (non-negotiable) with proficiency in OASIS
- 2+ years of combined acute nursing care (hospital-based) and Home Care experience
- Valid driver’s license and own vehicle required
- Active American Heart Association BLS certification
- Ability to work independently with minimal supervision
- Prior home health experience and proficiency with OASIS documentation
