Position Overview

As a Home Care Nurse, you will conduct client assessments, develop care plans, and collaborate with caregivers, clients, and families to ensure the highest quality of care.

Responsibilities

• Conduct initial and ongoing client assessments
• Develop, implement, and update individualized care plans
• Perform annual evals and skill assessments for Caregivers
• Monitor client health status and document care accurately
• Communicate effectively with physicians, families, caregivers, and office staff
• Provide education and support to clients and their families
• Ensure compliance with state regulations and company policies

Qualifications

• Active Massachusetts RN or LPN License
• Minimum of one year nursing experience preferred
• Home care, hospice, case management, or community health experience is a plus
• Excellent assessment, communication, and organizational skills
• Reliable transportation and valid driver's license
• Ability to work independently while maintaining strong teamwork
