Registered Nurse (RN) – Home Health

Location: South Dennis, Massachusetts

Duration: 13 Weeks

Pay Rate: $60 – $68/hour

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Registered Nurse (RN) to join a Home Health team. In this role, you will deliver high-quality, patient-centered care in a home setting. You will be responsible for conducting comprehensive assessments, developing individualized care plans, and supporting patients in achieving optimal health and independence.

Key Responsibilities

• Conduct comprehensive nursing assessments upon patient admission, including:
  • Physiological and psychosocial evaluations
  • Environmental assessments
  • Complete health history documentation
• Develop, implement, and update individualized plans of care
• Utilize HHRG values to determine visit frequency and support interdisciplinary care planning
• Monitor patient progress and ensure outcomes align with established clinical benchmarks
• Communicate changes in patient condition to physicians and the care team in a timely manner
• Participate in interdisciplinary case conferences
• Supervise Home Health Aide services and complete required documentation every 14 days
• Provide patient and caregiver education with clear and thorough documentation
• Maintain accurate, timely, and compliant clinical records
• Obtain, verify, and execute physician orders for all care services
• Coordinate patient discharge planning and complete all necessary documentation
• Refer patients to appropriate community resources as needed

Required Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Minimum of:
  • 1 year of nursing experience, or
  • 6 months of experience with a BSN
• Current CPR certification
• Successful completion of a Home Health medication competency assessment
