Details- Full-time and permanent- Opportunities for growth- Full, comprehensive benefits package (PTO, health insurance, life insurance, 401k, etc) Job Summary- Under the general direction of the Chief Clinical Officer (CCO) and the direct supervision of the (Associate) Medical Director, the night float advanced practitioner will provide after-hours care and access to comprehensive health care for the individual and family across all ages and genders within the context of Patient Centered Medical Home (PCMH) policies and procedures. The clinician will exercise individual initiative and independent action to ensure high quality and comprehensive outpatient medical care and after-hours coverage. The two central tenets of PCMH lie in team-based care and care coordination. The night float advanced practitioner is expected to function as an integral part of the care team, as a leader and an engaged team member.
