The CHA Pharmacotherapy Services program has been in place since 2006, growing incrementally from a modest warfarin Anticoagulation Management Service (AMS) at 1 primary care site to its current state with 20 clinical pharmacists and 3 specialized pharmacy technicians providing services in all adult primary care sites, several medical specialty sites, by telephone, and in patients' homes. An estimated 20,000 visits for medication management by our team are expected this year. The specific services provided include anticoagulation, diabetes, hypertension, hyperlipidemia, respiratory, pain, immunizations and behavioral health. The Pharmacotherapy Services team has been successful in improving the quality and safety of care across all service lines, while producing a high level of patient and provider satisfaction. Our team is involved in supporting population health improvement through chronic disease management in a variety of ways.

Job Purpose: The Practice Coordinator provides administrative and operational support in delivering high-quality patient care in accordance with established standards. This role ensures efficient practice operations and partners with clinical and service line leadership to drive performance, quality, and financial sustainability. Also, the role serves as patient care liaison with knowledge of departmental operations, recognizing clinically significant issues that require escalation, and prioritizing the communication of such expeditiously, accurately, and effectively for delivery of excellent service in accordance with established standards of care;providing high-quality service to patients in timely coordination of appointments and communication of related care needs, support of team projects, maintenance of patient care information.

Responsibilities:

Regularly provides support of pharmacotherapy services via telephone, written, or face to face communication with patients/caregivers in a professional manner, including clear documentation, and coordination of care. Ensures appropriate triage of patient care related to medication management services, including urgent escalation to a pharmacist or appropriate provider.

Provides outreach, follow-up, scheduling, and rescheduling of patients in the various pharmacotherapy services documented in electronic medical record (EMR).

Coordinates medical device applications related to pharmacotherapy care (e.g., point of care INR testing, continuous glucose monitor, insulin pumps); Documentation and communication of outcomes and care coordination with patient and care team.

Assists in the maintenance of team schedules and templates within the electronic medical record.

Maintains various sources of departmental data and information; analyzes data to provide general and specific information to leadership team, as required.

Participates in the coordination of Pharmacotherapy Services recruiting, hiring, and department onboarding for new employees or learners as needed.

Maintains various patient information lists (e.g., patient registries, patient follow-up, new patient referrals, media manager) in EMR.

Assists leadership in optimizing workflows and processes to maximize department operation efficiency based on department data, staffing and priorities.

Communicates professionally and effectively with internal and external colleagues and patients/caregivers in accordance with organization service standards.

Assists in ensuring compliance with all state and federal regulations, patient safety standards, and The Joint Commission accreditation standards.

Attends and participates in all required department meetings.

Qualifications / Requirements:

  1. High school diploma or G.E.D. certificate required.

  2. Associate or Bachelor’s degree preferred. Equivalent experience may be substituted for a degree.

  3. Proficiency with computer systems required, including web-based applications, spreadsheets/data management, scheduling, and human resource management.

Work Experience: Minimum of five (5) years’ in office management or customer service. Work experience in a medical setting is preferred.
