Since 2012 the Rural Medicine Program (RMP) team based in the Massachusetts General Hospital Division of General Internal Medicine has partnered with the Rosebud Sioux Tribe (RST) or Sicangu Oyate to provide medical care to people living on the Rosebud Reservation in South Dakota.

The partnership has expanded over time and now supports an array of community health programs managed by Rosebud Sioux Tribe Health Administration (RSTHA). To date over $12 million of health grants have been obtained by the RSTHA with the RMPs support.

The NPII will provide clinical care and care coordination services for individuals participating in a new federally-funded HCV elimination initiative pilot and will be a Key Personnel position reporting to the RMP Program Director and RST Public Health Medical Director. The NPII will play a key role in delivering trauma-informed culturally grounded services across the Rosebud Sioux Tribes integrated health system including detox behavioral health housing and mobile outreach. The NPII will work alongside peer support specialists and program staff to support screening diagnosis treatment adherence and linkage to wraparound services. The NPII will work with program physicians and the Medical Director to follow standing orders for HCV STIs and related conditions and to help optimize HCV case outcomes.

Job Summary

The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for the assessment and management of various populations of patients medical and/or surgical including diagnostic and therapeutic interventions development of appropriate plans of care and ongoing evaluation. May also perform additional duties such as precepting a small group of learners.

Essential Functions
-Provides direct care counseling and teaching to a designated patient population in the ambulatory inpatient operative and/or procedural setting.
-Performs complete histories and physical examinations.
-Orders interprets and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic laboratory and physical examination findings.
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
-Consistently provides high quality and timely documentation including admission and progress notes procedure notes operative notes and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.
