Pay Range: $29.00 – $47.00/hour Sign-On Bonus: $5,000 Case Manager Opportunity Collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans Coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions Monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care Case Manager Qualifications: Must be a licensed Registered Nurse (RN) or Licensed Social Worker (LSW/LCSW) New graduates are encouraged to apply!
