Schedule: Monday Friday, 8:00 AM 4:00 PM
Work Style: Hybrid 2 days remote, 3 days in the field or office
No nights, no weekends, no on-call!

Are you a compassionate and experienced Registered Nurse ready for a role with better balance and bigger impact? We're seeking an RN Case Manager to join our Senior Care Options (SCO) team, where you'll help older adults with complex needs navigate their health journeys and remain safely at home.

This is more than a typical nursing role. As a case manager, you'll build long-term relationships with members and families, create individualized care plans, and coordinate services to improve health outcomes. And with a consistent weekday schedule and a hybrid work setup, you'll have the flexibility and support you need to thrive.

What You'll Do:

• Conduct in-home clinical assessments and develop customized care plans
• Manage a panel of SCO members, ensuring high-quality, coordinated care
• Work closely with providers, care teams, and community organizations to meet each members goals
• Educate members and caregivers to support informed decision-making and wellness
• Document care and maintain compliance with program and clinical standards
• Engage in team-based planning, knowledge-sharing, and quality initiatives

What You'll Bring:

• Active RN license in Massachusetts (required)
• Bachelor of Science in Nursing (BSN) preferred
• Minimum 3 years of nursing experience (5 years preferred), ideally with home care or case management
• Strong organizational and communication skills
• Proficiency with Microsoft Office and electronic case management tools
• Reliable transportation and willingness to travel locally
