Responsibilities
- Administer prescribed medications and treatments per physician orders and nursing policies
- Assess residents, monitor for changes in condition, and report findings to the supervising nurse
- Document care provided in the electronic medical record with accuracy and timeliness
- Assist residents with activities of daily living (ADLs) and participate in rehabilitative activities as assigned
- Monitor vitals, perform routine tasks (e.g., wound care, glucose checks), and implement care plans
- Maintain a safe, clean, and therapeutic environment; observe infection control and safety protocols
- Collaborate with RNs, other LPNs, and interdisciplinary team members to ensure coordinated care

Requirements
- Active Massachusetts LPN license in good standing
- Minimum 1 year of recent experience in a long-term care or skilled nursing setting (preferred)
- CPR/BLS certification preferred
- Ability to work 40 hours per week in a long-term care setting
- Strong communication and documentation skills; comfortable with basic computer use
- Eligibility to work in the United States
