Case Manager
Position Summary
The Case Manager provides outreach, assessment and engagement to participants of the BSAS Low-Threshold Housing Program using a trauma-informed, person-centered approach. Support individuals who are in substance use recovery and experiencing homelessness to transition into housing, maintain housing, and ensure their other needs are met. Provide emotional support and psychoeducation re: coping skills and harm-reduction strategies. Complete documentation thoroughly and in a timely manner. Perform responsibilities professionally, in accordance with MHA’s Core Values (Respect, Integrity, and Compassion), policies, practices, program funding and applicable regulatory agency guidelines.
Pay Rate: $20 an hour
Open Shift: Monday through Friday 9am-5pm, some flexibility required
Key Responsibilities
- Coordinate and complete all required program entry documentation, including eligibility screenings, assessments, treatment plans, and required notices.
- Collaborate as part of a multidisciplinary team to develop and implement Individualized Action Plans (IAPs) using a strength-based, goal-oriented approach.
- Develop, implement, and monitor individualized support strategies, crisis plans, and behavioral interventions.
- Assist participants in securing and maintaining safe, stable housing, including landlord outreach, tenancy education, and housing advocacy.
- Provide hands-on tenancy support, including assisting with home moves, setting up utilities, address changes, and meeting housing standards.
- Support participants in accessing employment, education, training, benefits, and community resources to promote financial stability and independence.
- Promote community integration and skill-building in daily living, transportation, budgeting, self-advocacy, and recovery-oriented activities.
- Provide emotional support and harm-reduction counseling, coordinate and accompany participants to medical and behavioral health appointments, and respond to crises per agency protocols.
About the Division of Supportive Housing (DSH)
The Division of Supportive Housing (DSH) provides safe, stable housing and individualized wraparound services for individuals who have experienced chronic homelessness, many of whom are also navigating challenges related to substance use, incarceration, or trauma. DSH programs focus on long-term stability and help participants transition to independent, permanent housing. Through person-centered care, housing navigation, and community-based supports, we empower individuals to build a foundation for recovery, self-sufficiency, and a better quality of life.

Requirements
- Valid driver’s license, reliable vehicle, acceptable driving record, and sufficient insurance.
- Bachelor’s degree in human services or relevant field required.
- Requires 6 months of experience with individuals with mental health and/or substance use challenges or individuals with a history of housing instability.
- Must be at least 21 years old.
- Must be able to pass and maintain certifications in CPR and first aid.
