As a Recovery Support Navigator, you will work closely with a team of Recovery Coaches, Clinicians and support staff providing information and resources needed for an individual to obtain and sustain recovery from Substance Use Disorder.

Your role focuses on developing/identifying a set of goals unique to each individual on your caseload and working in conjunction with the system of care whenever possible. Utilizing your skills in crisis intervention and behavioral health, you will support clients in overcoming challenges while fostering resilience. People/communication skills, resourcefulness, and organizational skills are all key elements to this position.

Roles and Responsibilities- The Recovery Support Navigator (RSN) is a paraprofessional who provides care management and system navigation supports to engage clients as they present in the treatment system and support them in accessing treatment services and community resources including; bed finding, facilitating warm hand-offs upon referral to other services and supports, and navigating insurance issues.

Developing a set of goals and objectives in conjunction with the client, based on needs identified by the client and/or any care plans that exist for the client.

Supporting the client in understanding the treatment options available to them, including 24-hour programs, outpatient options, and medication-assisted treatment options.

Providing information about, and facilitating access to, community and recovery supports, including supports for families.

Assisting the client in accessing treatment services including, but not limited to:

Facilitating warm hand-offs to programs by maintaining relationships with addiction providers within the clients geographical area.

Navigating insurance issues with clients, including identifying and explaining in-network and out-of-network providers and advocating with providers and plans on the clients behalf.

Providing temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.

Delivering services on a mobile basis to clients in any setting that is safe for both the client and staff. Examples of such a setting include a clients home, an inpatient or diversionary unit, or a day program.

Provide linguistically appropriate and culturally sensitive recovery support navigation that embraces the physical and intellectual challenges and their chosen pathway to recovery.

Qualifications- Must have a Bachelors degree in social work or a related field. Substance use disorder treatment/case management experience preferred. Must have a valid driver's license, and reliable transportation.
