- Provide physical therapy services under the supervision of a Physical Therapist
- Assist with treatment plans to improve patient mobility and function
- Deliver care to patients in a home health setting
- Monitor patient progress and report updates to the supervising therapist
- Educate patients and caregivers on exercises and therapy techniques
- Travel between patient homes within the assigned service area
- Maintain accurate documentation and follow established care plans
Charting system: Homecare Homebase (experience preferred)Active Massachusetts PTA license requiredHome health experience requiredCurrent BLS certification required
