• Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home
• Community-based nursing opportunity centered around care planning, continuity of care, and service coordination
• No bedside nursing responsibilities required
• Hybrid structure combining community patient visits with remote follow-up and care coordination work

• No bedside nursing demands
• No nights, weekends, or call responsibilities

• Monday-Friday schedule
• 8:00 AM-4:00 PM hours
• Three community-based days weekly
• Two remote workdays weekly
• No nights
• No weekends
• No call responsibilities COMPENSATION & BENEFITS
• Base salary: $84,000-$108,000 annually based on experience

• Develop individualized patient care plans
• Coordinate community-based services and ongoing care needs
• Conduct patient visits within community and home settings
• Manage follow-up and care coordination remotely

• Active Massachusetts RN license REQUIRED
