General Description
The AWC Service Navigator coordinates services within the Agency with Choice Program, participates in relevant training, and reports to The AWC Coordinator.
General Qualifications
Bachelor’s degree in social work or related field (preferred) and two to five years of experience, in the field, working with individuals with developmental and intellectual disabilities and with their families. Other qualifications include working knowledge of resources, benefits, and services; good communication and organization skills; ability to make sound clinical decisions; and a commitment to working as a team member.
Job Responsibilities
- Protect the rights and dignities of individuals supported and extend those rights and dignities to family members and guardians.
- Understand and implement through work with families the Principles of Family Support.
- Understand and implement through work with individuals and families the frameworks for “Charting the Life Course”.
- Assist the AWC participant/family in developing a vision for support, planning for the ISP, and supporting the development and writing of requested assessments to establish goals and objectives for the upcoming year and attending ISP meetings.
- Develop with participant/family AWC budget by reviewing need areas and following DDS’ guidance for both direct service hours and financial assistance, completing for new fiscal year and then ongoing to meet the needs of the participants. Write and submit for review and approval, as needed over 5K plans as applicable.
- Promote community connections by helping to identify and share contact information for local, generic community resources, and community organizations that may be helpful to the participant/family to address their support needs, including recruitment of potential Direct Service Staff, and providing opportunities for new experiences and relationships.
- Maintain regular communication with all parties to discuss progress and assist with problem solving and help arrange assistance in times of emergency/crisis. Determine with each participant/family, on an ongoing basis, the appropriate amount of contact. At a minimum, provide direct communication with the participant/family at least monthly via phone or email and quarterly conduct in-person meetings.
- Update DDS Service Coordinator with participant’s progress and/or any emerging needs.
- Provide support with all activities related to the employment process including creating job descriptions, posting positions, helping participants/families identify resources and places to recruit direct service staff, participate in interviews of identified applicants, help facilitate the hiring process, negotiate and establish rates of pay, and complete performance evaluations. Work with participant/family to develop more independence with these activities over time based on their capacity.
- Provide initial training regarding the AWC program and expectations for new DSPs working with participants; subsequently provide ongoing communication and support to all staff.
- Provide opportunities for participants/families to network with each other to share experiences and ideas, as well as to connect with other services offered by the AWC Provider that may be relevant and helpful.
- Provide education and oversight regarding fraud and abuse for participants and families.
- Maintain standards related to individuals’ and families’ privacy and HIPAA regulations including obtaining releases of information.
- Review The Arc of Bristol County’s Agency with Choice (AWC) agreement with participant, family, guardian if applicable, complete agreement with new participants and annually with all participants. Review Program requirements with participant and family.
- Approve staff time in ADP, ensuring proper documentation for services has been submitted.
- Review, record, and submit payables related to participant’s financial assistance following all applicable Family Support Department and AWC Guidance procedures.
- Update AWC database with utilization of funds and hours upon submitting payables and/or approving time for staff.
- Follow all agency and department specific policies and procedures, as well as DDS’ January 2023 Guidance for AWC.
- Maintain the timeliness of submission and ensure accuracy of all required documentation such as data sheet, progress notes, database referral forms, mileage reimbursement, etc.
- Perform other duties as assigned.

The employee is regularly required to use hands or feel objects, tools and controls; reach with hands or arms, stoop, kneel, talk or hear. The employee is required to stand, walk and sit and may occasionally be required to lift/transfer up to 50/70 lbs alone or occasionally up to 200 pounds with assistance from staff. Pay: $23.00 - $24.00 per hour
